Background:Nolvadex (dexormonal therapy) is a nonsteroidal antiestrogen that decreases estrogen levels in postmenopausal women. It has been shown to increase testosterone and to reduce the incidence of breast cancer. In addition to its use in treating breast cancer, it is also being investigated as a possible therapeutic agent in other conditions that affect estrogen receptors.
Methods:A systematic literature review was performed, with the aim of identifying studies that have evaluated the effect of Nolvadex on estrogen-related adverse events in postmenopausal women. Eligible studies were published in English and in Caucasian populations. Six studies were included: four were published in Spanish and two in English, with a total of 517 men and 1,053 women (mean age, 55.1±4.6 years). Three were conducted in the United States, with a total of 9 studies investigating menopausal symptoms. Two of these studies included women who had received tamoxifen and/or Nolvadex or an estrogen receptor antagonist, while the others were conducted in menopausal women undergoing surgery.
Results:Five studies investigated the effect of Nolvadex on the menopausal symptoms of postmenopausal women. A total of 2,742 postmenopausal women completed the study, of whom 2,831 were in menopausal status and were taking Nolvadex. The overall incidence of menopausal symptoms was 5.5%, which was higher than the incidence found in Caucasian women. The overall incidence of breast cancer was 7.1%, compared to a 4.2% incidence in Caucasian women. There were no differences in terms of the incidence of other adverse events between menopausal and postmenopausal women.
Conclusion:This review suggests that Nolvadex may be a useful agent in the management of menopausal symptoms in postmenopausal women. However, it must be carefully considered and taken into account when prescribing Nolvadex to a woman in this setting.

RESULTS - Results of the systematic search were retrieved from the Cochrane Library, using an index search of ‘‘. Articles that included ‘Tamoxifen’, ‘Nolvadex’, ‘Nolvadex,’ ‘Tamoxifen’, ‘Nolvadex,’ or ‘Tamoxifen’ were excluded. Two studies with a total of 463 women, which included menopausal symptoms, were included in this systematic review. The results of the search were evaluated independently by two researchers. The data from the included studies were pooled. The results were presented as the summary table. In addition to the findings of the systematic review, a literature review was performed. The conclusions of the systematic review were expressed in terms of the relationship between Nolvadex and the risk of breast cancer. The risk of breast cancer was significantly increased by Nolvadex in menopausal women (RR 1.23; 95% CI 1.08–1.42). A total of 11 studies were included in the systematic review (2,746 women; 5,898 women). The overall incidence of breast cancer was 5.5% in women taking Nolvadex or an estrogen receptor antagonist, compared to 1.2% in women taking an estrogen receptor antagonist alone. The overall incidence of breast cancer was also significantly increased by Nolvadex in menopausal women (RR 1.23; 95% CI 1.08–1.45).
